(Hawaii Relief Program) Maui Economic Opportunity has been contracted to administer the Hawaii Relief Program. Eligible applicants may receive up to four months of financial assistance, including: -Up to 4 months of rent or mortgage (up to $6,000 per month) -One time security deposit (actual housing deposit amount up to the maximum of $6,000) -Up to 4 months of utility assistance ($2,000 per month per utility source) -One time Utility deposit (up to $3,000 per utility source). Utility payments may include electricity, gas, or water expense, or a combination of these utility sources. Water expense may include sewer service fees. Payments are made directly to landlords, mortgage lenders, or utility providers. That includes being able to talk to a Registered Nurse. (SFMNP, Senior Farmer's Market Nutrition Program) The Senior Farmers' Market Nutrition Program (SFMNP) in collaboration with our transportation department, provides seniors transportation and vouchers to farmer’s markets across the island in an effort to increase community access to fresh, local food. (ATV, YWCA of Kauai Alternatives to Violence) Provides psycho-education and support designed to interrupt the cycle of violence. Assists clients with anger management. Encourages, teaches and advocates non-violent behaviors. Available as a resource in the community for speakers and presentations.

(Youth Shelter, Youth Shelter - Maui) Provides 24-hour emergency shelter for youth 10 - 17 years. Offers individual, family and group counseling. Provides substance abuse prevention, educational services, including tutoring, transportation to the public schools and GED preparation. Organizes cultural, recreational and community activities. Provides after care follow-up to youth and their families returning to a home situation. Provides case management services for independent clients.

A training awards program designed specifically for persons currently working with, or planning to work with, older adults. These awards will provide tuition assistance for workers who enroll in credit or noncredit courses and certificate programs, which enhance their knowledge and skills in areas related to aging. Selected programs can be either in local classroom settings or online. Applications are accepted on a rolling basis.

(DVSF) The Domestic Violence Services for Families program provides counseling services for offenders, survivors and their children who have been referred by DHS programs. Services are provided primarily through group and individual counseling. Serves the Windward and Central area of Oahu. Groups area available after hours in the evening.

(Easter Seals Hawaii Home and Community-Based Services, Easter Seals Hawaii Honolulu Services Center Adult Day Health Program) Adult Day Health (ADH) focuses on providing a small group environment that fosters growth, and challenges the adult participant to increase independence in a variety of community settings. Emphasis is placed on individual choice, allowing adults to pursue opportunities that match their interests and skills while promoting their dignity and independence. The program provides daily respite for individual and family caregivers as well as provides participants with educational, social, health and recreational opportunities. ADH participants have a full-range of activities to choose from including: the option to go on daily excursions, enjoy art programs, volunteer in the community, attend computer classes with the aid of assistive technology, learn money skills, participate in weekly hula classes or give back to the community through recycling programs. These items are received by the local offices by donations from others and through our TLC Catalog. Support Groups: The office is a clearinghouse of information on local self-help and support groups that provide emotional and educational help. (H-HEAP, Low-Income Home Energy Assistance Program-Energy Crisis Intervention-Kalihi-Pala, LIHEAP ECI, ECI) Hawaii Home Energy Assistance Program (H-HEAP) Energy Crisis Intervention (ECI) is designed to help families keep their utilities (gas or electric) on, especially those households that are most vulnerable, such as the elderly, the disabled, and households with young children. H-HEAP primarily assists households by applying an energy assistance grant directly to the energy provider on behalf of the eligible household. Applications are accepted year-round. (Women Infants and Children Nutrition Program) The Special Supplemental Nutrition Program for Women, Infants and Children (WIC), is a federally funded program which provides Hawaii residents with nourishing supplemental foods, nutrition education, breastfeeding promotion and health and social service referrals. The participants of WIC are either pregnant, breastfeeding, or postpartum women, and infants and children under age five who meet income guidelines and have a medical or nutritional risk. Nutrition and Health Education: WIC will answer your questions about what to eat for a healthy pregnancy and help you plan family meals and snacks. Education and support for moms who breastfeed: WIC has specially trained staff to answer your breastfeeding questions. Ask for more information about our breast pump loan program. The Golden West Chapter’s evidence based wraparound model of care combines coordinated, customized medical care and community and home-based services and support. The Chapter’s proven Excellence in ALS Care Initiative encompasses all key elements of our Care Services program which are focused on ensuring proactive, expert multidisciplinary care. In addition to providing direct services, we simultaneously engage the ALS community in activities that accelerate the search for effective treatments and cures. The Chapter’s Care Services program is an evidence based “wraparound model of care” that combines fully coordinated and individualized medical care with community and home-based services and support. This model is clinically proven to help people live longer and better with ALS, as it addresses the medical, social, emotional, and financial challenges they face. The Chapter’s approach in the state of Hawaii is unduplicated and tailored for each client and their loved ones. For example, Care Managers help clients and their loved ones access information and resources in their communities, connect with clinicians who specialize in ALS, obtain durable medical equipment and augmentative communication devices available through the Chapter’s Equipment Loan Program, understand and navigate insurance benefits, provide guidance around home modification, help find professional caregivers, and connect with others living with ALS. And if people with ALS and their loved ones have concerns about anything else related to this disease, our Care Managers are there to help.
