(HCHP, CBCM-RS, Community Based Case Management- Recovery Services, Ohana Project Services, Shelter Plus Care) Health Care for the Homeless Project (HCHP) is offered at our Kaaahi street clinic and Kohou street clinic. The Kaaahi street clinic is at the Institute for Human Services (IHS) Homeless Shelter for Women and Families. Primary Care health services are provided to single women and families who are guests of the IHS Homeless Shelter, single men, and community members: Including adult medicine, chronic disease management, immunizations and flu shots, infant and well-child check ups, physical examinations, psychiatric counseling and medication management for adults, and women’s health.

Counsels parents to decrease the stress of parenting. Answers questions about child's behavior, growth and development. Provides information on mental health services and other child and family resources. Distributes Keiki O Hawaii, Parent Resource Directory, and Teddy Bear Post which lists services for parents.

(MST, Multisystemic) This program is a unique, goal-oriented model that provides comprehensive treatment that serves chronic, violent delinquents ages 12 years old to 18 years old. It is a family focused, in-home and community-based treatment program that has been the focus of several major research studies, and demonstrated clinical and cost effectiveness for youth with complex emotional, social and academic needs Multisystemic (MST) is a time-limited, intensive family and community based treatment that addresses serious anti-social behavior in juvenile offenders ages 12 years old - 18 years old. Program addresses the factors associated with delinquency across the youth’s settings (e.g. family, peers, school, neighborhood) and uses the strengths of each system to foster positive change. At least one adult/parental figure must be willing to assume long-term parenting role and participate with service providers for the duration of treatment.

Scholarships are given as a reimbursement for the tuition of college-level courses in early childhood education, for Professional and Career Education for Early Childhood (PACE) classes taken if converted to college credits, or for direct assessment fees in obtaining a Child Development Association (CDA) certificate. (K9 Kokua) K9 Kokua provides direct outreach to dogs of the houseless on Oahu and Maui, persons affected by domestic violence, including kupuna and veterans facing hardship. Services include veterian care resources, wellness checks for dogs, owner education, assistance through housing transitions. Fostering available for person affected by dometic violence. K9 Kokua is an all volunteer agency. Volunteers are accepted for everything from writing thank you cards to direct outreach services.

(Food Bank) Hawaii Foodbank operates two AIB-certified warehouses on Oahu and Kauai where we receive, inspect, sort, store and distribute food to our communities. Food is sourced from manufacturers, growers, retailers, wholesalers, Feeding America, individual and corporate food drives and federal commodities where our staff and volunteers prepare these items for safe and efficient distribution through our partnership of more than 200 agencies including food pantries, soup kitchens, homeless shelters, domestic abuse shelters and rehabilitation centers. Food distribution is also through food pantries in local communities and the Ohana Produce Plus program, which provides fresh produce and assorted groceries to anyone in need while supplies last. (MOCA) Mentally Overcoming Challenges and Adversity (MOCA) is a safe space for all youth ages 12 years old - 24 years old to talk about mental health challenges and get help locating resources in their community. MOCA provides youth and young adults with: 24/7 access to support and resource referrals, non-judgmental and anonymous support via trained staff, and connection to nationwide resources, including mental health support, and services. (St Francis Hospice) Nationally accredited by The Joint Commission, St. Francis Hospice, the state’s first hospice program, provides compassionate care and comfort for terminally ill patients in their homes, in nursing homes, and at our inpatient facility in Nuuanu. We make each moment count, enhancing the quality of life for patients, and their families and friends. St. Francis Hospice also has one of the most advanced bereavement services programs, offering support groups, walks in shopping malls, and other activities for families who have lost a loved one. St. Francis Hospice provides a specially trained, interdisciplinary healthcare team that may include a medical director, nurse, social worker, chaplain, hospice aide, volunteer, and bereavement counselor. The St. Francis Hospice care team focuses on each individual patient’s needs to ensure comfort and the best quality of life for our patients and their families.

Provides support to anyone grieving the death of a friend or family member. Holds one-on-one sessions with staff as well as volunteer assistance offered by individuals who have experienced the grief process. Offers a six week bereavement support group which is held three times annually, providing an opportunity for healing as experiences are shared and information offered. Maintains a lending library of books and videos.

(Nanas House Family Centers - Waimea, Neighborhood Places of Kauai) In partnership with a community Ohana Board and other community agencies, provides family support and family preservation services to the families in defined areas of Kauai. Incorporating a community family center model, we provide a healing and nurturing environment in which intervention services are interwoven with prevention activities. Services include information and referral, emergency food pantry, emergency clothes closet, parent skills classes, youth development programs, individual support for expecting and new parents, assistance with food stamp applications, job information services, outreach activities to engage hard to reach families, individual and family counseling, tobacco treatment and community activities and cultural awareness activities.

Our Cancer Rehabilitation Program (CRP) is a comprehensive program that provides outpatient rehabilitation to cancer patients and survivors. Whether newly diagnosed or finished with treatment, we can help decrease pain and enable individuals to safely and effectively regain independence. Our goal is to help patients love their life as they did before cancer. For those who want to increase their independence and fulfill rehab goals, the CRP may be the right path. Patients work with a team of cancer rehabilitation specialists, including physical therapists and exercise physiologists, to create a program tailored specifically towards improving quality of life.
