(HCHP, CBCM-RS, Community Based Case Management- Recovery Services, Ohana Project Services, Shelter Plus Care) Health Care for the Homeless Project (HCHP) is offered at our Kaaahi street clinic and Kohou street clinic. The Kaaahi street clinic is at the Institute for Human Services (IHS) Homeless Shelter for Women and Families. Primary Care health services are provided to single women and families who are guests of the IHS Homeless Shelter, single men, and community members: Including adult medicine, chronic disease management, immunizations and flu shots, infant and well-child check ups, physical examinations, psychiatric counseling and medication management for adults, and women’s health.

(LEI) LEI is a HTA-sponsored workforce development project created by ClimbHI to inspire Hawai’i’s youth to select careers in the Hawai’i visitor industry by providing them with the necessary means to achieve success. From the student’s perspective, LEI represents an opportunity for Leadership, Exploration, and Inspiration, and from the hospitality industry’s perspective, LEI provides an opportunity to Lead, Expose, and Inspire Hawai’i’s youth. The program began in 2011 with 300 Hawai’i high school students spending a day learning about the hospitality industry and the value of graduating from high school and going on to pursue a post-secondary education. During the course of the day’s program, the students meet, learn, and experience job opportunities that the hospitality industry can provide. Students also receive advice on filling out job/college applications, resumes, personal finance, and support resources. (Meal Delivery - Hawaii Meals on Wheels) Medical meal (diabetic, renal, chopped, pureed, etc.) delivery service to homebound kupuna. Meals are frozen or hot, as needed. Frozen meals delivered once a week to your home and is a week's worth of meals. Hot meals are daily during weekdays, including all holidays. Both hot and frozen programs offer medical meals. Helps frail elderly and disabled individuals preserve their independence at home by providing nutritious meals and regular personal contact. Clients will have to have limited mobility; not be able to meet certain criteria of daily living i.e. difficulty dressing themselves, unable to cook or shop on their own. For private pay and sponsored services, A Hawaii Meals on Wheels staff will check in with their physician on their health status.

Sex Addicts Anonymous (SAA) offers referrals to 12-step recovery meetings for people who have a desire to recover from sexually compulsive or addictive behavior. Meetings are located all over the world with the majority being in North America. Meetings are held seven days a week at all times of the day. Meeting formats include face-to-face, telephone, and web-based options. SAA does NOT offer telephone counseling nor is it a “hotline.” Interested parties will be referred to a meeting in his/her area or to a telephone/web-based meeting for assistance.

(IOP) The Intensive Outpatient Program (IOP) is an intensive psychotherapeutic program designed to treat substance use disorders and co-occurring mental health disorders. Services include group therapy, individual counseling, and toxicology testing. Sessions are often done as a group. Clients go home each night, rather than living at a treatment center. Clients attend IOP groups 3-5 days per week, for a total of 9-15 hours per week.

Hawaii Circle of Care for Dementia provides Caregiver education and training in both group and one-to-one settings; Case management and volunteer assistance for individuals living alone with dementia; New resources for those at an early stage of dementia.
