This CDC-recognized program is a year-long structured lifestyle change program that includes dietary coaching and moderate physical activity, all with the goal of preventing or delaying the onset of type 2 diabetes among those who are at high risk for diabetes. The program consists of 16 core sessions over the course of the first six months, followed by less-intensive classes held monthly for the rest of the year-long program. The goal of the program is to cut one's risk of developing diabetes in half by losing 5-7% of their body weight through healthier eating and engaging in 150 minutes of physical activity each week. This program also includes shopping tours, cooking classes, exercise, on-site gardening, and stress management sessions.

Angel Flight West is a nonprofit, volunteer-driven organization that arranges free, non-emergency air travel for children and adults with serious medical conditions and other compelling needs. Our network of 1,500+ pilots, as well as commercial airline partners, throughout the 13 western states donate their aircraft, piloting skills, and all flying costs to help families in need, enabling them to receive vital treatment that might otherwise be inaccessible because of financial, medical, or geographic limitations. Ambulatory patients needing medical transportation to the 13 Western States; Alaska, Arizona, California, Colorado, Hawaii, Idaho, Montana, New Mexico, Nevada, Oregon, Utah, Washington and Wyoming may be eligible. (Homeownership Education and Coaching Program - Kona Office) The Hawaii HomeOwnership Center's (HHOC) lifetime membership prepares buyers for the home purchase through classes and individual coaching. Classes address the general concepts about preparing for homeownership and the buying process; individual coaching provides support to identify buying resources and creating an action plan to become mortgage ready. Support after our members become homeowners is available through workshops and confidential meetings.
