Staff will also provide information and referral sources when needed. Designed to improve the quality of life for families, care receivers, and caregivers by offering a continuum of quality care that is flexible and individualized in order to prolong the care giving relationship. The program actively assists the care receiver and the caregiver in implementing an individualized, strengths based service plan on how to reduce stress and link with the appropriate community resources. The program also provides supportive counseling.
