(West Hawaii Processing Center - Ka'u) The federal Supplemental Nutrition Assistance Program (SNAP) provides crucial food and nutritional support to qualifying low-income and needy households, and those making the transition from welfare to self-sufficiency. Question: Will DOH accept temporary driver’s licenses and state ID cards from applicants ordering replacement vital records? Answer: YES, temporary driver’s licenses and state ID cards are acceptable. Question: Are there alternative documents that can be used to order a birth, death, or marriage certificate if all government-issued IDs are missing? Assesses and treats swallowing disorders. Provides mobility training or wheelchair skills. Develops adaptive techniques and custom-designed equipment. Provides speech theraphy. Computer therapy for visual, perceptual, and cognitive training. Stroke Support Group meets every first Tuesday of each month from 10:00 am - 11:30 am at the Rehabilitation Hospital of the Pacific. Everyone (patient, family, caregivers, etc.) is welcome, there is no membership fee.

(Therapeutic Crisis Homes) The Therapeutic Crisis Homes (TCH) program provides youth the opportunity to stabilize with trained TCH parents who provide them with support and guidance they need to return to their families safely. Services are offered to each youth based on his/her individual needs. Youth are provided with individual counseling and family counseling with the ultimate goal of reunification with their families within 3 to 7 days. Crisis Home Parents provide a safe environment for youth ages 5 years old - 17 years old to stabilize their emotional and physical situation in time of crisis. Services included support, guidance and individual as well as family counseling to stabilize the situation so that youth can reunite with their families within 3-7 days. Trained staff professionals also help to develop an after-care plan for families and referrals to public and private follow-up services.

(MEM) Making Ends Meet provides emergency financial assistance for individuals who experienced an unexpected life event that resulted in a financial setback. Assistance for essential needs such as rent, utilities, transportation, etc. up to about $500. No general financial assistance. No long-term financial support available.
