The Golden West Chapter’s evidence based wraparound model of care combines coordinated, customized medical care and community and home-based services and support. The Chapter’s proven Excellence in ALS Care Initiative encompasses all key elements of our Care Services program which are focused on ensuring proactive, expert multidisciplinary care. In addition to providing direct services, we simultaneously engage the ALS community in activities that accelerate the search for effective treatments and cures. The Chapter’s Care Services program is an evidence based “wraparound model of care” that combines fully coordinated and individualized medical care with community and home-based services and support. This model is clinically proven to help people live longer and better with ALS, as it addresses the medical, social, emotional, and financial challenges they face. The Chapter’s approach in the state of Hawaii is unduplicated and tailored for each client and their loved ones. For example, Care Managers help clients and their loved ones access information and resources in their communities, connect with clinicians who specialize in ALS, obtain durable medical equipment and augmentative communication devices available through the Chapter’s Equipment Loan Program, understand and navigate insurance benefits, provide guidance around home modification, help find professional caregivers, and connect with others living with ALS. And if people with ALS and their loved ones have concerns about anything else related to this disease, our Care Managers are there to help.

The national Disaster Distress Helpline (DDH) is a program of the U.S. Substance Abuse and Mental Health Services Administration (SAMHSA) administered by Vibrant Emotional Health (formerly the Mental Health Association of New York City) that offers 24/7 crisis counseling and emotional support to anyone in the US/territories experiencing distress or other behavioral health concerns related to natural or human-caused disasters. Callers and texters are connected to trained counselors from a network of independently-operated crisis centers across the country. Services are available to people who are Deaf or hard of hearing via the DDH Videophone for American Sign Language (ASL) users. The DDH also offers peer support communities through private Facebook groups where people across the U.S. impacted by disasters can connect with others from similar backgrounds/experiences for mutual aid and support during recovery. (Hawaii Homeownership Center - Honolulu Office) The Hawaii HomeOwnership Center's (HHOC) lifetime membership prepares buyers for the home purchase through classes and individual coaching. Classes address the general concepts about preparing for homeownership and the buying process; individual coaching provides support to identify buyer resources and create an action plan to become mortgage ready. Support after our members become homeowners is available through workshops and confidential meetings.

(VLA) The VLA project focuses on disabled and elderly veterans and family members who are residents of the state of Hawaii and who want assistance with filing benefits claims with the U.S. Department of Veterans Affairs. Also offers referrals to volunteer attorneys through VLSH's pro bono program for legal advice and direct services for: Family Law, Tenant Rights, Collections and Bankruptcy, Estate Planning/Probate, Re-Employment and Community Service Program (Oahu only).

Kauai Island Utility Cooperative (KIUC) provides electric services, connection and repair. Payment Arrangement Plans of up to 3 months are offered for members having difficulty retiring a past due balance. Financial Assistance programs are available through partner agencies.
