Program helps achieve specific goals that includes but is not limited to obtaining and maintaining Transitional Housing, Supportive Permanent Housing and long-term Permanent Housing and will continue to provide Housing Services with Evidence-Based and Low-Barrier Service Models, as well as Coordinated Data Entry System utilizing (VI-SPDAT) and the Housing First Model Approached. Case Manager/Care Coordinator coordinates resources and community-based programs that helps with housing and other needs they may have. Maintains bi-weekly meetings to process and follow-up activities, conducts team meetings with providers including probation officers, mental health, substance abuse and medical.
