Hospice is a holistic philosophy of patient centered care. The hospice program of care focuses on the whole person-body, mind, and spirit. Hospice recognizes that a patient who is dealing with the end of life seeks and deserves comfort in every aspect of his/her life. Hospice care is provided through an Interdisciplinary Team (IDT) consisting of a physician, nurses, nure-aides, social workers, spiritual counselors, nutritional and other therapists and specially trained patient-care volunteers who offer support through the challenges of living with a terminal illness to the patient and family seven days a week, 24 hours a day.

(Summer EBT) Summer EBT, also called SUN Bucks, is a new and permanent program to provide benefits to households with school-aged children to purchase food during the summer. Households will receive $177 for each qualifying child, to be spent during the summer months. Eligibility must be established annually. The SUN Bucks Program for Summer 2025 operating period will be from May 31, 2025 through August 3, 2025. When will SUN Bucks expire? Unused benefits expire 122 days after the date benefits are issued. Please use the benefit within that time frame. Once expired, the benefit cannot be reloaded. For children who automatically qualify, the SUN Bucks EBT card with the $177 benefit already loaded will be mailed out by FIS starting the week of May 19th via USPS. Households should anticipate that it may take 1-2 weeks for delivery. Cards are sent to the mailing address based on the child’s school records, or the household's mailing address per their SNAP or TANF case if the household participates or has participated in those programs. Wilson Homecare is a state-licensed home health agency providing personal care to skilled nursing from 3 hours per shift, once a week, to daily 24 hours care at the clients home. Care assessment will be made by Nurse Care Coordinator to build the appropriate care plans which can be assistance with Activities of Daily Living (ADLs - bathing, dressing, toileting), running errands, visits to the doctor, medication management, meal prep, or light housekeeping. We also offer skilled nursing in home which can included medication management, wound care, vent care, up to life support.

(HAP) The Housing Assistance Program (HAP) serves senior adults, aged 60 years or older, who need housing counseling and assistance to find affordable housing. Priority is given to those with the greatest economic and social needs and those facing homelessness. HAP helps to improve and maintain their self-sufficiency and remain in the community by increasing their accessibility to affordable and supported housing and by empowering them to make difficult life transitions. The program provides housing counseling and linkages to housing resources, assistance to remain on housing wait lists, updates of the Oahu Housing Guide list/database, and education and advocacy to promote the development of new affordable, supportive, well-designed rental housing. Serves current and former foster youth in Hawai‘i who are transitioning out of the foster care system, into adulthood. (PPA) The Partnership for Prescription Assistance brings together America’s pharmaceutical companies, doctors, other health care providers, patient advocacy organizations and community groups to help qualifying patients who lack prescription coverage get the medicines they need through the public or private program that’s right for them. Its mission is to increase awareness of patient assistance programs and boost enrollment of those who are eligible. It is a service available by phone and web site that acts like a clearinghouse, checking all known discount or free services to try and meet prescription needs by eligible callers or web inquirers.
