Traumatic Brain Injury Rehabilitation Program

Our Brain Injury Program provides acute rehabilitation for patients who have sustained traumatic brain injury. We provide a comprehensive and individualized plan using an interdisciplinary team approach, working closely with patients and their families/caregivers to address specific needs and goals. Our patient-centered care model is focused on patients to maximize their potential. The REHAB staff also includes Certified Brain Injury Specialists (CBIS) who have the expertise and knowledge to work with special needs following a brain injury. We help patients schedule a follow-up appointment with their physicians in our Physicians Clinic so they can continue their rehabilitation therapy in our outpatient NeuroTrauma Recovery Program (NTRP). The NTRP is designed for patients recovering from a traumatic brain injury and provides comprehensive rehabilitation assessment and service for impairments in cognitive, psychosocial and physical abilities due to a traumatic brain injury. Caregivers and family members are provided with a three-hour Caregiver Education and Training (CET) prior to discharge, meeting with each member of the interdisciplinary care team to get hands-on training.

(Da Box) DA BOX is a Community Supported Agriculture (CSA) program. The Food Basket purchases Hawaii Island grown produce from our local farmers and resells to customers island-wide, keeping produce affordable and increasing small farm viability. DA BOX was created to both support local farmers and to provide affordable access to fresh, Hawaii Island grown produce. Each week of produce consists of a minimum of seven items, generally one to two fruits and five to six vegetables. The program website also provides free access to recipes, a "food for thought" community blog, and a "meet the farmers" section to introduce the community to the local farms and farmers, as well as a weekly newsletter.

The federal Supplemental Nutrition Assistance Program (SNAP) provides crucial food and nutritional support to qualifying low-income and needy households, and those making the transition from welfare to self-sufficiency. The SNAP processing center provides public assistance, food stamps (SNAP) and financial assistance, to eligible individuals and families. They handle new applications and ongoing assistance. A paper copy of the application can be downloaded, printed, signed and submitted via US postal mail or dropped off in the drop box. -CDC program is a year-long program that includes dietary coaching and moderate physical activity. -consists of 16 core sessions over the course of the first six months, followed by less-intesive classes held monthly for the rest o the year-long program. -currculum classes are augmented with free cooking demos, demonstration of simple exercises, opportunities for activities like walking with Health Educator and Community Helth Worker, referrals to other resources in the community, relaxation techniques, and incentive gifts. -goals are to prevent or delay the onset of type 2 diabetes among those wo are at high risk for diabetes, cut one's risk of developing diabetes in half by losing 5-7% of their body weight through healthier eating and engaging in 150 minutes of physical activity each week.
