Whether disease start at home or abroad, are curable or preventable, chronic or acute, or from human activity or deliberate attack, CDC responds to America’s most pressing health threats. CDC is headquartered in Atlanta and has experts located throughout the United States and the world. To accomplish our mission, CDC conducts critical science and provides health information that protects our nation against expensive and dangerous health threats, and responds when these arise.

Implements control measures necessary to decrease and prevent infectious disease, including provision of vaccines to health care providers. Waianae Coast Comprehensive Health Center's case management and homeless outreach provides outreach and intensive case management services to high risk patients of the Health Center referred by their primary care provider or other provider in the Health Center. Also provides health care via mobile van services to the homeless living along the Waianae Coast. Outreach is provided at the beaches Monday afternoons and Tuesday mornings.

Shelter of Wisdom rents homes which are shared by our men. Men live in double rooms with shared living areas and house chores. We provide intensive case management until our men are accepted into permanent housing. We provide household furnishings for their new homes, which are donated to us by the community. Men must maintain their sobriety, hygiene, medications, meal preparation, laundry, housekeeping and behavior.

(HCAP STEM) The Ha Initiative: Creative STEM After-School Program is a free Science, Technology, Engineering, and Math (STEM) after-school program for youth in grades 2-8. The program provides year-round tutoring, mentorship, and fun STEM activities in a safe after-school setting at five STEM Exploration Center locations on Oʻahu.

The School Readiness Project provides developmental, hearing, vision screening and case management for children ages two to five years old in the community, at preschools or at selected events. Workshop and training presentations are provided to parents and professionals on the importance of early childhood health and development to emphasize the importance of children being kindergarten ready.

Parents And Children Together provides a safe, nurturing facility that offers survivors of domestic violence and their children housing for up to 12 months. Services include housing education, childcare referral, and employment counseling.
