Six month time period given to provide victims the opportunity to gain focus and perspective of their personal goals and career choice.

(SSVF) HOPE Services Hawaii Supportive Services for Veteran Families (SSVF) promotes housing stability among veterans’ families with very low income who reside in or are transitioning to permanent housing. The SSVF Program may be able to provide the following services as they relate to housing stability: Outreach services, case management services, assistance obtaining VA benefits, assistance obtaining other public benefits and needed services within the community, limited financial assistance that is necessary to stabilize housing, assistance with housing searches, financial planning as it relates to stabilizing housing, health care services, daily living services, transportation services, fiduciary and payee services, and legal services.

(Adult Services Team) Hale Naau Pono provides case management and outpatient therapy.

(MEO) Provides up to 6 months of case management that includes developing an individual service plan, budget preparation, assistance with job retention and job skill development, and other counseling. In addition to case management clients may receive up to a maximum of $3500 in rental assistance. This is structured for a six month period with the first month's rental not to exceed $1200.

(SSG Fox SPGP) SSG Fox Suicide Prevention Grant Program (SSG Fox SPGP) is designed to assist service members, veterans and service-connected family members with resources through outreach, suicide prevention services, and connection to VA and community resources. A comprehensive range of services includes individualized, compassionate case management; assistance with transportation, shelter, and childcare; access to mental health and substance abuse treatment options; help with navigating healthcare choices; and housing placements services. Additionally, the SPGP offers financial literacy education, referrals for legal assistance, peer specialist support, and resource navigation.

Provides housing, case management, and nutrition services to Hawaii residents on Island of Oahu with disabilities, including HIV/AIDS and have very low income. Housing services include transitional shelter, short-term rent assistance, utility assistance, security deposit assistance, and rental subsidy programs. Case management is provided in conjunction with housing assistance. Nutritional services includes grocery distribution, home delivery to the house bound, and twice weekly hot lunches.

Parents And Children Together (PACT) offers safe, compassionate, and confidential support in three counties for youth and families coping with serious behavioral, social, and emotional challenges. Our evidence-based therapeutic models focus on family networks and relationships, school or job performance, peer connections and relationships, and community networks of support. (HCHP Kohou Clinic, CBCM-RS, Community Based Case Management- Recovery Services, Ohana Project Services, Shelter Plus Care) The Kohou street clinic provides assistance for Health Care for the Homeless Project (HCHP) such as Case Management and Homeless Outreach island wide on Oahu.

Provides primary health care including medical and psychiatric services for eligible veterans with stable, chronic health problems or minor acute illness. Provides social services, case management services, and information and referral to appropriate resources.

(HIHR) Provides Transitional Housing Program, Permanent Supportive Housing Program, Kitchen and Pantry Food Service Programs for the homeless. Provides assistance especially those with mental illnesses, substance abuse disorders, dual diagnosis, physical health and other disabilities, prioritizing the chronically homeless. Program helps achieve specific goals that includes but is not limited to obtaining and maintaining Transitional Housing, Supportive Permanent Housing and long-term Permanent Housing and will continue to provide Housing Services with Evidence-Based and Low-Barrier Service Models, as well as Coordinated Data Entry System utilizing (VI-SPDAT) and the Housing First Model Approached. Case Manager/Care Coordinator coordinates resources and community-based programs that helps with housing and other needs they may have. Maintains bi-weekly meetings to process and follow-up activities, conducts team meetings with providers including probation officers, mental health, substance abuse and medical.

Provides a wide range of support services for persons living with HIV/AIDS, their family members and significant others. Assists clients in applying for benefits and other AIDS-related services. Sponsors case management services, counseling, support groups, recreational activities, meal programs and vocational counseling. Provides financial assistance with rent/mortgage and other expenses when funds are available. Specialized services for Native Hawaiians, and those with drug or housing issues. VOLUNTEERS WELCOME! Accessing all aspect of medical care including vision, dental and drug assistance. Qualifying for public and private aid programs for housing and financial assistance, home health services, essential transportation, etc, Counseling and referral services including mental health, harm reduction and return to work programs, disability claims and a fully stocked food pantry on-site.

The program also offers intensive case management services known as Pathway of Hope (POH). POH is for families who are ready to break the cycle of intergenerational poverty and move towards self-sufficiency.
