Adult Day and Elder Care Program

Home and Community Based Services

(HCBS) Adult Day Health in combination with Community Learning Services Group supports participants through a set of services on site and community based. Residential Habilitation provides individualized supports that assist the resident with the acquisition, retention, or improvement in skills related to living in the community. Chore Service provides services to maintain the home as a clean, sanitary and safe environment in order to ensure the participant’s health and welfare. Community Learning Services, Individual supports participant’s integration in the community. Personal Assistance/Habilitation includes a range of assistance or habilitative training services provided in the participant’s own home or family home to enable a participant to acquire, retain and/or improve skills related to living in his/her home. Training and Consultation assist unpaid caregivers, paid service supervisors and/or paid support staff in implementing the goals and outcomes developed from the person-centered planning process included in the Individualized Service Plan. RCH started its Developmental Disabilities Domiciliary Homes program in 1977. The homes were built by RCH with funds from the U.S. Department of Housing and Urban Development (HUD). The homes are licensed and monitored by the State Department of Health. Currently, RCH operates two domiciliary homes with a capacity of housing and providing services for up to four residents in each home. The homes are managed by Domiciliary Home Managers who provide a family environment and support the residents in developing their independent daily living skills through integration in the community. The program consists of a highly structured skill developmental training program focused on personal care, social integration, and systematic positive behavior changes. The program is complemented with a day activity program and transportation services.

Ohana Care - Oahu

(CFS Gerontology Program - Ohana Care) Caregivers who provide the primary care to individuals who are disabled, frail, and/or isolated residents on Oahu, who are 60 years of age or older, on an as needed basis are eligible for case management, counseling, and training services. Training topics may include: ADL/ADL limitations, caregiver’s role, disease prevention and remediation, access to available resources, stages of disease and options/ referrals in legal or financial related matters, RCI-REACH, and Arthritis Foundation related training. Staff will also provide information and referral sources when needed. Designed to improve the quality of life for families, care receivers, and caregivers by offering a continuum of quality care that is flexible and individualized in order to prolong the care giving relationship. The program actively assists the care receiver and the caregiver in implementing an individualized, strengths based service plan on how to reduce stress and link with the appropriate community resources. The program also provides supportive counseling.

Just in Time Training

Compassion for Cancer Caregivers provides a 2-hour virtual session on creating a network of support for a cancer caregiver and/or patient. Topics Include: The Wall of Silence, Walking in Their Shoes, Actions Speak Louder Than Words, A Journey of 1,000 Miles Begins with a Single Step. Virtual and in-person meetings are offered. The site for in-person meetings are mutually determined.

Dba Kohala Home Health Care

67-1125 Mamalahoa Highway, Kamuela, HI 96743

Provides home care services to homebound patients: skilled nursing, occupational therapy, medical social services, home health aides, physical therapy and speech therapy. Offers intravenous therapy (i.e., fluids, antibiotics, nutritional fluids), wound care, teaching patient and/or family member appropriate skills to care for self or patient. Aides assist with bathing, ambulation, feeding, light housekeeping.
