Currently uses van, Care-A-Van outreach specialist to travel to beaches/parks and other areas on Kauai where homeless people live or congregate. Waianae Coast Comprehensive Health Center's case management and homeless outreach provides outreach and intensive case management services to high risk patients of the Health Center referred by their primary care provider or other provider in the Health Center. Also provides health care via mobile van services to the homeless living along the Waianae Coast. Outreach is provided at the beaches Monday afternoons and Tuesday mornings.
