Parents And Children Together provides support through home visits and works in partnership with families. Our parenting curriculum promotes safe and nurturing parenting and access to resources about healthy child development. (Hui Malama Ola Na Oiwi) We provide disease prevention and health promotion services through advocacy, accessibility, and education. We offer Case Management Services (such as home visits to conduct blood pressure checks, glucose checks, assistance with applications for medical insurance), and referrals to other health-related agencies. We also provide Health Education (Diabetes, Hypertension, Nutrition, and Tobacco Cessation), Traditional Healing Workshops and Classes, as well as one-to-one services by referral. Specialty Transportation provided to medical-related appointments (based on needs and availability)- for individuals restricted to a wheelchair or gurney.

Operates an adult day health program which provides therapeutic services and treatments including nursing care, recreational therapy and social services. Designed for persons at risk for long term care placement. Accepts volunteers 16 years old or older, to assist in transporting residents, general office duties, music leaders, craft assistants, active games, religious services, pet therapy, ethnic groups, sewing craft projects, etc.

We support, educate and advocate for choice at the end of life. We support individuals as they navigate their way through the health care system. We support legislation relating to choice in dying as well as work to stop legislation that restricts choice. We advocate for those whose end-of-life wishes are not being followed. We provide state-specific advance directives and information on how to complete these documents.
