Our Brain Injury Program provides acute rehabilitation for patients who have sustained traumatic brain injury. We provide a comprehensive and individualized plan using an interdisciplinary team approach, working closely with patients and their families/caregivers to address specific needs and goals. Our patient-centered care model is focused on patients to maximize their potential. The REHAB staff also includes Certified Brain Injury Specialists (CBIS) who have the expertise and knowledge to work with special needs following a brain injury. We help patients schedule a follow-up appointment with their physicians in our Physicians Clinic so they can continue their rehabilitation therapy in our outpatient NeuroTrauma Recovery Program (NTRP). The NTRP is designed for patients recovering from a traumatic brain injury and provides comprehensive rehabilitation assessment and service for impairments in cognitive, psychosocial and physical abilities due to a traumatic brain injury. Caregivers and family members are provided with a three-hour Caregiver Education and Training (CET) prior to discharge, meeting with each member of the interdisciplinary care team to get hands-on training.

(Commodity Supplemental Food Program, CSFP, Hawaii Food Basket Kupuna Pantry) Kupuna Pantry works to improve the health of low-income elderly persons at least 60 years of age by supplementing their diets with nutritious USDA foods. Food packages include a variety of foods, such as nonfat dry and ultra high-temperature fluid milk, juice, farina cream of wheat, oats, bran cereal, rice, pasta, peanut butter, dry beans, canned protein meat, poultry, or fish, and canned fruits and vegetables.

(SSVF) Supportive Services for Veteran Families (SSVF) provides outreach and case management service to improve housing stability by providing supportive services to eligible Veteran families residing in or transitioning to permanent housing. Supportive Services include housing search assistance, landlord mediation, and temporary financial assistance. SSVF also provides information and referrals to assist participants with obtaining VA benefits and other public benefits, which may include health care services, personal financial planning services, and legal services.

(H-HEAP, Low-Income Home Energy Assistance Program - Energy Crisis Intervention - Central, LIHEAP ECI, ECI) Hawaii Home Energy Assistance Program (H-HEAP) Energy Crisis Intervention (ECI) is designed to help families keep their utilities (gas or electric) on, especially those households that are most vulnerable, such as the elderly, the disabled, and households with young children. H-HEAP primarily assists households by applying an energy assistance grant directly to the energy provider on behalf of the eligible household. Applications are accepted year-round. Assists in the location and recovery of missing children. Connected to 49 mainland state clearinghouses as well as the National Center for Missing and Exploited Children. Traces children and their abductors through connection with these databases and other public records as well as coordination with the efforts of the FBI and police departments of all states. Child Protective Services, Interpol, the U.S. Department of State and countries who are signatories to the Hague Convention. The police should always be notified first in the event that a parent suspects a child has been abducted. Accepts volunteers to assist with basic administrative assistance, special events and graphic designing.

(Act 278) The Mediation Center of the Pacific has been contracted to administer the Eviction Mediation Pilot Program. This two-year pilot program requires early and free mediation between landlords and tenants before eviction for nonpayment of rent. The program runs February 5, 2026 - February 4, 2028. Tenants have 10 calendar days to respond to an eviction notice. If a tenant requests mediation in 10 day notice period, then the landlord must participate before filing for eviction. Mediation sessions are online via Zoom or in person, per request. For landlords, visit the website for more information and to submit a Landlord Intake Form to start the process. Organized in 1979, the Hawaii Pacific Gerontological Society (HPGS) is a nonprofit organization dedicated to improving the quality of life of Hawaii’s older adults. We promote the understanding of the aging process, supportive services, and legislation that help people age with dignity and grace. It is the largest voluntary, multidisciplinary group in the State committed to dialogue and discussion, networking and information sharing on issues and policies affecting older adults in this geographical area. Currently, more than 400 older adults, caregivers, gerontology educators, medical and social service practitioners, health care providers, agency personnel and organizations from all sectors of the community make up the membership. Members can attend the meetings held three times a year.
