-Must have a Waianae Coast Comprehensive Health Center (WCCHC) Primary Care Provider (PCP) -Must be 18 years old or older -Have a nutrition-related chronic disease (ex. diabetic, high blood pressure, etc.) and/or food insecurity -Have Medicaid insurance or SNAP/EBT -Living along the Leeward Coast of Oahu

Participants can use vouchers at Makeke Farmers and Green markets or shop online with free delivery during the 12 month program.
