Eligibility

Representative Payee consumers (persons who have a Severe Mental Illness/Severe and Persistent Mental Illness diagnosis) must be referred by Ohana Health Plan Community Care Services ((888) 846-4262) or by direct referral from the Social Security Administration.

The Representative Payee program provides money management and budget counseling for individuals receiving Supplemental Security Income (SSI) / Social Security Disability Insurance (SSDI) benefits who are unable to manage their finances on their own due to a diagnosable mental illness.
