Eligibility

-Individuals with proof of Medicaid insurance and a diagnosis of Alzheimer's or Dementia will qualify for the Memory program provided by KCFCCS. -Veteran's and their spouses can qualify for the Veteran's program. Special consideration will be given to individuals who are not eligible for either program, but have shown a need for care services and meet financial requirements.

Hours

Tuesday-Friday 8am-4pm Assessment Services are provided as needed, as determined at time of assessment, and can included evenings and weekend coverage

Potential recipients and/or a family representative must call to schedule an assessment. Services are determined via a care matrix, and recipients of KCFCCS can receive up to 20 hours of free/at-cost care as determined by the matrix. Re-evaluations are do

We provide in-home care services which include companionship, light housekeeping, cooking, supervision, Activities of Daily Living; including bathing, toileting, transfer assistance, dressing, feeding, and grooming, and various other duties as determined by the assessment. Kokua Care Foundation Charitable Care Services (KCFCCS) provides these care services at no charge to qualified individuals, and at-cost services for individuals who are unable to qualify for the program, but have shown the need for service.
