Eligibility
-Must have a Waianae Coast Comprehensive Health Center (WCCHC) Primary Care Provider (PCP)
-Must be 5 years old - 13 years old
-Have a nutrition-related chronic disease (ex. diabetic, high blood pressure, etc.) and/or food insecurity
-Have Medicaid insurance or SNAP/EBT
-Living along the Leeward Coast of Oahu

We want to understand how prescriptions for fresh produce can improve health. Participants can use vouchers at Makeke Farmers and Green markets or shop online with free delivery during the 12 month program.
