Application process

Appointment with Outreach Worker required. Need to have proof of age (driver's license, birth certificate or state ID) and evidence of Hawaiian ancestry.

Provides social service and health program for Native Hawaiians aged 60 and over. Serves lunch meal each day. Provides transportation within limited geographical distance to and from site. Provides escort service to medical appointments and other related services. Offers friendly visiting, telephone reassurance and referrals to other community agencies.
