Medically underserved persons are defined as: "Individuals who, because of economic, geographic, cultural or physical barriers, lack access to primary health care services." The Association is comprised of fourteen health centers operating in fifty locations in the islands including Bay Clinic, West Hawaii Community Health Center, Hamakua Hamakua Health Center, Community Clinic of Maui, Hana Health Center, Molokai Community Health Center, Lana'i Community Health Center, Kalihi Palama Health Center, Kokua Kalihi Valley Health Center, Waikiki Health Center, Waimanalo Health Center, Waianae Coast Comprehensive Health Center, Koolauloa Community Health and Wellness Center and Kauai Community Health Center.
