Eligibility
-US citizens and some aliens who are admitted for permanent residency may qualify.
-Households must meet specific eligibility requirements and provide proof of their statements about household circumstances.
-Most able-bodied adult applicants must meet certain work requirements.
-All household members applying must provide a social security number or apply for one.

(Hale Na`au Pono, SNAP Outreach, Supplemental Nutritional Assistance Program Outreach, Supplemental Nutrition Assistance Program (SNAP) Outreach, Waianae Coast Community Mental Health Center - Hale Naau Pono - SNAP Outreach) The federal Supplemental Nutrition Assistance Program (SNAP) provides crucial food and nutritional support to qualifying low-income and needy households, and those making the transition from welfare to self-sufficiency.
Hale Naau Pono SNAP outreach program assists you in-person or over the phone in completing and submitting an application or a renewal form to a Department of Human Services Processing Center. In addition, the Outreach workers help recipeints to maintain their benefits and provide follow up (in most situations) on case status if needed.
