Referral by Department of Vocational Rehabilitation. Need to bring identification, birth certificate, TB clearance, physical exam, psychological or psychiatric reports and other testing as needed.

Assesses the vocational potential of disabled persons through work samples, situational observation, diagnostic interviews and psychometric tests. Provides information and recommendations for vocational counseling and rehabilitation plans. Conducts physical capacity evaluation, case work and counseling, psychosocial assessment, occupational skills assessment and work experience.
