Eligibility
Persons 60 years old or older, frail and vulnerable as defined by the Hawaii County Office of Aging criteria and living at home may be eligible.

Application process
Referring party calling for a frail elderly person should call the Aging and Disability Network at (808) 961-8600 first and they will refer to the appropriate agency. Services for Seniors, Inc.:(808) 935-1144.

None, if qualified.

Services for Seniors provides case management, performs health assessments on the disabled elderly in their homes with or without a caregiver present, links to existing services in the community and assures home is safe, purchases service and/or equipment, provides respite to caregiver, and coordinates services with other agencies.
